I found my router (Actiontec Rev D) dead yesterday afternoon at 2:30pm. 
I called Fios customer service. They went thru the tests and confirmed it was dead. Said they would ship one out. It (Rev E) arrived via UPS at 10:30 this morning.  Now that's customer service. This forum always has the negatives

 so when there is a positive experience it deserves to be noted.
