I ordered an upgraded modem in late March (Confirmation number: {edited for privacy}.
The Modem has arrived and was successfully installed.
There was no return slip included in the box to be used for the return of the old modem.
I have called now three times, asking for a return slip. I have received three promises but no return slip. The latest promise was made on April 4 (Reference number: {edited for privacy}). I still do not have a return slip.
Could someone help me?

Be sure to hold on to the old one just in case. But most of the time they don't want the old one back.
