You should have received a post card and/or email with a way to track your reward status on verizon.com/rewards
It took me almost a full complete 90 days before I saw mine via UPS.
If you ordered fios online did it show the reward card as you checked out and give you an option to choose it? When I did my order a few years back I had to actually choose the card and add it to my cart as I checked out. I dont know if they still do that or not.
If they don't call you back, do you have a facebook account? The customer service on their facebook page might be able to look into it for you. They are usually pretty fast at responding on there. Always are with me.
