Returned Equipment but Still Have Note on Bill About Unreturned Equipment

When moving, I returned my old HD DVR as requested using the prepaid UPS label.  I was told by the UPS clerk to keep my receipt, as he heard equipment frequently was not taken off the bill.  Well it looks like that may have happened to me, as there was a warning on my bill that I still need to return my equipment.  How can I make sure I am not billed and/or sent to collections?
