I've forgotten my username and password, and have since lost the postcard they came on for the Rewards Status Login page (http://www.verizon.com/rewards/); is there any way I can view my credentials or reset them?

I am not sure, but when I went there it gave to me some hints.
#1 The user name is the billing account number if on DSL / Is the master order number if on FIOS.
#2 The password is the last name listed on the account.

I have entered the order number from when I ordered my service and my billing account number along with my password. Neither are accepted. If the so-called "Master Order Number" different from the ordernumber I was given when I ordered service? I ordered my service in March 2012, called in June regarding not receiving it, and still have yet to receive my "rewards card".
