Effective date and self-setup not working

I have an active ONT in place and can connect to my router, but there are no options to complete my install.  The Verizon fios app has an effective date past when I requested service activation.  I’ve been on customer support for hours and cannot get through to anyone.  The service apps and virtual chart are zero help.  Does anyone have any suggestions on how I can get service activated?

Can you go to activate.verizon.com then choose Order Number as your identification method for the sake of good measure? Screenshots are helpful here, but please redact out your personal information such as order numbers, address, account number, and etc.
