Channels missing that we're being billed for

We just started FIOS triple play this month.  Some of the channels such as HBO that we are paying extra for and appear on our bill are not available on our tv, and it says we are not subscribed to them on the tv.  We have the Ultimate package but Starz disappeared as well.  What's going on?
