Here are tweets I've gotten back from @FIOSTV over the last year about this app that was initially demoed by Verizon in August of 2010:
1/24/12 - will be limited channels - was just reviewing the final release candidate today
10/31/12 - I have it on my ipad if it helps. (my reponse - when will it be out 1 month, 2 months ? to which he replied) most def
It's been 19 months since the first demoed the app and it apparently has been working for many months for internal users. I know the holdup is probably something with content rights, but seriously how long can that take to work out? Even if the app is released with very limited channels it's at least something to show the customers that it is here and Verizon is working on getting more channels to add. I also don't understand why there is such an issue with the content providers for watching in your own home on your WIFI network only. It's as if the ipad is an extra TV and it will only increase viewership without anyone losing money.
Has anyone heard anything about the status of this? It also drives me crazy that Verizon won't come out and provide an update on this even if they say it's 6 months away.

Well? No reply from Verizon....what does that mean? Do I look to go to Optimum that does have the app?

It seems VZ would dearly like the channels themselves to "do the deed" eg; HBO/Go, etc.... EPIC FAIL, Verizon. Other providers already have implemented this, VZ's EMPTY PROMISES for almost 2 years leave a bad taste. Let's face it, VZ just won't (or can't) deliver on it's commitments.
