more frequently, the dvr recordings are the appropriate length but are completely blank...just a black screen.  Also, the recoding that are recorded correctly have the first 5-10 minutes all jumpy on the screen.

Any ideas how to fix this?  Seems like the disk need the equivalent of a chkdsk/f

Contact FiOS tech support and have the DVR replaced. Not knowing the exact model, but it sounds like you have a drive that is getting ready to go. They could possibly reload, or reset the box with diagnostics, but that will not fix hardware that is about to quit. Watch all your programs and get a new DVR.
