DVR - not resuming where i left off

I noticed this over the past couple of months.  The dvr is not always resuming where i left off.  i start watching and say, "havent i seen this already!?".  Not sure why this stopped working right.

 

Also, why are the recorded episodes sorted from most recent down?  it should be oldest first and stay that way. I can sort these, but then the next time i go in, it defaults back to recent first. Strange. i always have to go all the way to the bottom to see the most recent.  Why would we want to go farther vs shorter distance to see episodes in order?

What model DVR do you have?

How are you resuming plan of in progress recordings?
