Three ways. Contact a Customer Service Rep or Verizon.com or take the box to a local presence store.

I was told I could return the STB to any Mail Boxes Etc/UPS Store (it's the same thing). They took the set top box and remote control, made a copy of the front page of my Verizon bill and that was that.
Actually they are not the same thing. Mail boxes Etc, is just an authorized ups drop off location but they are not a true UPS store. Only the true "UPS Store's" are authorized Verizon drop off locations. But that is the easiest way. Take the box and go down to a UPS Store and let them know it is for Verizon and they will take care of it for you. Just keep the tracking number as your receipt and records
