I have tried with no success to set up a trouble ticket...Ran the test and it indicated that I needed a tech to come out....The app will not allow me to place contact info in the area. Please help, my internet is up and down and all around for a few weeks now, just getting worse.

Have you tried the Verizon Troubleshooter? You can use this to fix and report issues with your Verizon Phone, FiOS TV, or Internet Service, as well as to schedule a repair, here is the link:

Yes, the troubleshooter does not allow you to place in contact information. It asks "How do you wish to be contacted and there is a blank space below with no place to enter information
