Has anyone got this working? I can't seem to. I created a test file by cutting and pasting the example and carefully making the necessary modifications. I get a "page not found" message for my response file. If I comment out the url command, I get a thank you page. But the email never arrives.

While I do not use the form2mail script, I can tell what I noticed about e-mail,
It does not come in right away.
If it does not, after - let us say at least 24 hours, please post.
I am not saying that e-mail takes that long - And if so, why...

I gave up on the script. I found this service on the web and it works great: webspamprotect.com.
With luck, I'll never to deal with Verizon "technical support" again.
