Digital Voice Account Manager Missing Menus

My Digital Voice Account Manager is missing the "Voice Mail Settings" menu. Also, when I call the voicemail and try to access the menus - it is missing there as well. I have been on the phone with support and they do not know what to do to fix the issue.
Does anyone know what I need to tell them to do???

A trouble ticket needs to be sent to the group that handles the FVAM features.
