What happened to my upgrade rebate??

Months ago I upgraded my service to triple play, faster internet, self install with the promise of a $200 rebate in the form of a prepaid Visa card and a $50 self install rebate.

Never heard another word about it from Verizon, nothing in the mail, no email, nothing.

Anything that I can do to find out what has become of it?
