$14.50 service charge

I am trying to find out why I have a service charge on my account. My FIOS plan was setup incorrectly & I had to call multiple times to attempt to get it corrected. I think one of those calls has been charged as a $14.50 fee. I can not find anything on the web site as to when fees are charged so I do not understand why some customer service calls have fees and others do not. 

Also, it seems that my bill was only adjusted for my 1st month and then went back to the incorrect plan. How else can I contact Verizon because I do not want to be charged again for service calls when they are making the mistake.
