Can not redeem Amazon Prime / free Echo offer

I am not able to redeem my free offer for a year of amazon prime and echo. I have talked to customer service agents 4 times and spent many hours on hold and no one has been able to help me. When I click on the link to redeem on amazon it says my offer has already been redeemed. Can anyone assist me with this issue?
