Never received $50 Amazon gift card

This was part of the promotional deal that hooked me into FIOS. I signed up. in Dec 2018. However, a little more than a year in now, I have not received any indication of getting the $50 Amazon gift card. I see from the forum I'm not the only one. Please help me by rectifying this error.
