Phantom orders

In the past 3 days someone or something has changed my plan and ordered newer service. My TV plan was changed on 2/3 to news and variety. NO Hallmark no HGTV. My wife blew a fuse. After an hour on the phone it was turned around. Tonight I received a thank you for FIOS for my new service with an additional charge of $66 additional per month.

There are two of us in the household I didn't do it and neither has my wife. I don't think my dog did it since you need thumbs to change on the TV.

WHAT GIVES?
