Verizon lost my equipment on return

I returend equipment in my own box using a label that Verizon e-mailed to me and even though I have proof of delivery from UPS, Verizon is claiming they never got the equipment, even though the proof of delivery says one of their guys signed for it. All I have is proof that UPS delivered--I can't find a copy of the e-mail with the original label. UPS can't get it for me because the label originated with Verizon, but my lawyer needs the specific address that it was sent to. Any ideas on how to solve this mess that I've been dealing with for THREE MONTHS???
