When did I "sign" a new contract?

I had a contract back when it was only $79.99. I never "signed" a new agreement either verbally or digitally yet I have "12 months" left on my "plan" ??? I've tried to find my "contract" so I can review it but can't seem to find it on the site map or by searching.

 

Currently I pay $30/mo for unlimited calling yet I don't have a phone. A year ago I tried to get this removed from my bill but nothing was resolved. I pay for the use of a set top box and full price for everything so I have no "plan."
