Dear Billing Agent,

I've received the bil dated July 22,15  Account # {edited for privacy} .total amount is $18.32.

I do believe that some of services i did not sign up for posted on my billing are not correct.

Nonlisted Service :     4.25

Fios Inside Wire Maintenence :    $8.99

Activation fee:          $16.66

Surcharge :    $7.48

Total of these services :     $37.38

These services have been charged for three bills cycles :     $ 37.38 X 3 =  $112.14

Please, review my account and adjust this amount and credit back to my account.

PS:     When I  ordered on line.    i did not sign up these services and Activation nd Onstalation are free according to the agreement online they posted.

I hope to be your customer for ever if you calculate evrything correct not  like this situation
