I have a FIOS Internet installation and requested Ethernet be run from the ONT to my router. My question is will Fios run it to where my router is if its not in the same room as my ONT, also will they provide the cable and would it be cat 6 cable?

Usually a cat 5e, so if you want cat 6 you might want to buy it ahead of time, and run it yourself. A FiOS Tech may run it based on the distance, but they don't fish through walls or anything unless the walls are already open.. so if you want a nice neat looking job, then I would run it myself

They have asked for conduit before - so if there is conduit already there, probably, if not then that might be questionable. Not sure if any vz tech's post here themselves, but I would imagine that would be an individual case basis, they'd probably have to eye ball it and give you an opinion.

If you're getting a speed tier over 100Mbps, then probably.
If not and there is coax where you want the router, probably not.
If running cat5 makes more work for the tech, then no.
As for cat6, no.
In other words, don;t create more work for the tech.
Run the cable yourself in advance of the install.

Most of the time drop ceilings are not a problem. Can take a tech no more than 5 minutes to do 30 feet. Enclosed walls and between floors are the problem.
