Not getting 75/35 service I was provisioned for (and am paying for) despite optimizations

When I run third-party diagnostics they say I'm getting 50/20.   Verizon's speedtest says I'm getting 50/39.  But I was supposed to be provisioned for 75/35.  I've run Verizon's optimizer to increase SendBuffer and Receive Window. But the speed is the same before and after optimization.

When I run the Verizon diagnostics they find a 45 Mbps T3 link.  How can I get 75 Mbps from a 45Mbps pipe?
