It will depend on the desired results and what system you currently have (Mac, Win, Myth). For example, with a QAM Tuner, you'd be able to receive all the local stations and Public stations in your viewing area only. You'd be able to change the channel with your TV software. However, if you want to watch Premium channels, it will have to be routed through the STB. You'd change the channels with the remote to the STB. You could get an IR Blaster to send IR signals to the STB to change the channel via the computer. Of course there would be a delay of 1 to 2 seconds when changing channels.
I think Hauppauge makes a PCIe card that supports a CableCard. This would essentially make your computer into a true DVR. It is suppose to be designed for the MediaPCs.

You record programs with it. So will the PC tune to the correct channel or do you have to manually change the channel to what you want to record. I have never used a set top box to record on Media center.

The only cablecard system in place that I know of is the ATI ones. But if you built you MCE PC yourself it will not work. The company that makes cablecards has to certify the PC for it to work. So the only ones it works on are OEM PCs like HP, and Dell. And even then, not all models are supported. I almost picked one up before I found out that little bit of info.
