Replacement DVR sent. Can't Authorize/Register Mobile Because It Has Reached It's Limit

Our DVR could not connect with the Fios TV app so support decided it could not connect with our home network and sent us a replacement DVR. DVR was setup successfully but we can't register my iPhone because it reached it's limit in 12-month period (see photo). How do I resolve this? Thanks in advance.image
