Join Mobile + Home Rewards sees old Fios account. I can't change it!

I had a Fios account and closed it and went to Optimum. I have since rejoined Fios under again and have a new account and username. 

When I try to join Mobile + Home Rewards in the Verizon app on my cell phone, it automatically populates the old Verizon Fios username into the Fios Username field, and it's grayed out and I cannot change it to the new account username. 

How can I fix this so I can enter the new Fios account username? I've tried to chat with a rep on the website, but nobody ever answers. I tried calling and can't get through. I've been trying for about 2 months now! Frustrating!
