If you only have one "set-top" DVR box, should you be getting charged 19.99/mo for "Multi-room DVR"?

Yes.  The Multi-room DVR gives you the ability to schedule a recording from other STBs and play back to other STBs.  It is that extended capability that you are paying extra for.  The basic DVR only allows recording and playback on that DVR.

I don't think you understand my point.

I don't have ANY other STBs.  None.  Just the one.  How can I have multi-room functionality if I only have one TV?
